What is in vitro fertilisation and how is it done?

In vitro fertilization (IVF) is a complex fertility treatment that involves multiple steps:
1. Ovarian Stimulation: Hormonal therapy is used to induce a woman’s ovaries to generate many eggs rather than the single mature egg that occurs during a monthly cycle.
2. Regular Monitoring: Monitoring: Using blood tests and ultrasound images to check follicle development and hormone levels, the woman’s response is continuously observed during the ovarian stimulation process.
3. Retrieval of egg: To induce the final maturation of the eggs, a trigger shot is given once the follicles have grown to their ideal size. Transvaginal ultrasound-guided follicle aspiration is a minimally invasive method used to collect the eggs about 36 hours later.
4. Fertilization: After being recovered, the eggs and sperm are mixed together in a lab dish and fertilized. Conventional insemination involves combining sperm and eggs, whereas intracytoplasmic sperm injection (ICSI) involves injecting a single sperm into each mature egg.
5. Embryo Culture: To enable them to develop, the fertilized eggs—now embryos—are cultivated in a laboratory environment for a few days.
6. Transfer of Embryo: One or more embryos are chosen for uterine transfer after a few days of culture. Usually, a thin catheter that is placed through the cervix and into the uterus is used to do this.
7. Pregnancy Test: To find out if the IVF process was successful, a pregnancy test is carried out 10 to 14 days following the embryo transfer.

Precautions for a woman of age post 50 yrs considering IVF

According to Dr. Aruna Kalra, Director, Obstetrics and Gynaecology, CK Birla Hospital, Gurugram, “A woman of advanced age should have a thorough health evaluation prior to undertaking IVF in order to assess any pre-existing medical issues that could affect her ability to conceive or become pregnant. In addition, a comprehensive evaluation of the ovaries, uterus, and other reproductive organs should be carried out. This evaluation should include tests to determine the ovarian reserve, uterine health, and hormone levels. Screening for age-related risks is crucial to ensuring the safety of the mother and any potential offspring, addressing issues like gestational diabetes and hypertension. Pregnancy complications, chromosomal abnormalities, and higher rates of miscarriage are among the increased risks associated with being pregnant at an advanced age.”
In addition, women who are thinking about IVF should have a thorough conversation with their fertility specialist about all of the possible risks, restrictions, and expected results of the process, including the possibility of multiple pregnancies, the likelihood of success etc.

Eligibility criteria for IVF

– Those who have failed at earlier reproductive treatments could opt for IVF. The decision-making process, however, may be influenced by details of earlier therapies, such as any underlying reasons for infertility and the outcomes of those treatments.
– Sufficient egg quantity and quality, indicating adequate ovarian reserve, for a successful IVF procedure.
– A healthy uterus capable of supporting a pregnancy.
– A male partner who can provide enough healthy, fertile sperm, or, in an emergency, use donated sperm.
– lack of illnesses or other problems that could raise the risks of IVF or pregnancy significantly.
– Mother should not be more than 50 yrs of age , and father should not be more than 55 yrs of age
